While not completely necessary for HSP claiming, there are certain forms you will be very familiar with, and use on a daily, or near daily basis.
We have integrated the most common ones into the client's file to save you time and energy!
Follow the steps below to Generate HSP related forms:
1. Go to Clients > Clients File
2. Click Hearing Services > Generate Form
3. From the drop down select the type of form you wish to generate:
Maintenance Agreement
Client Relocation
Private Services
Statuary Declaration
Wishes and Needs Tool (WANT)
4. Fill out the relevant fields
5. Click Preview if needed
6. Click Download
