Questionnaires allow you to ask a set of defined questions for each client, perfect for intake forms or health evaluations. Once questionnaires have been created they can be undertaken by the client as many times as required and exported as a CSV file.

Create a questionnaire template

1. Click on Setup, then Settings
2. Click on Questionnaires 
3. Click Create Questionnaires 
4. Enter the name of the questionnaire, and set the number of questions to be asked then click Create
5. Create the questions, and set the requirements, then click Next
6. Enter the answers, then set a score for each answer, then click Next
7. Enter the results based off the scores, then click Finish

📝 Once the questionnaire has been created, it can be filled out for your clients.
